Abstract

본 연구의 목적은 특실병동에서 근무하는 간호사들의 의사결정 경험을 기술하기 위한 것으로, 연구질문은 ‘특실병동 간호사들은 어떻게 의사결정을 하고 있는가?’이다. 본 연구를 위해 2007년 7월부터 11월까지 특실병동 간호사 10명을 대상으로 심층면담을 이용한 근거이론적 방법을 적용하였다. 면담자료의 기록 및 보관을 위해서 녹음 하였으며, 필사한 후 이를 토대로 Strauss와 Corbin(1998)이 제시한 방법에 따라서 분석하였다. 연구결과는 다음과 같다. 본 연구에서 특실병동 간호사의 의사결정 경험을 나타내는 중심현상이자 핵심범주는 ‘깊이 있는 통찰로 유연하게 대응하기’였다. 이러한 중심현상의 인과적 조건은 ‘특별한 대접을 요구하는 환자’였다. 그리고 특실병동 간호사의 의사결정 경험의 특정한 속성인 맥락적 조건은 ‘다양한 진료과의 환자간호’,‘동료 간호사간의 경험의 차이’,‘환자중심의 특실병동 분위기’로 도출되었다. 중심현상인 ‘깊이 있는 통찰로 유연하게 대응하기’를 위한 작용·상호작용을 촉진하거나 억제하는 중재적 조건에는 ‘간호사의 문제 해결방식’,‘환자와 보호자의 태도’,‘의사와의 관계’,‘정보의 접근성’등이 포함되었다. 의사결정과정에서 특실병동 간호사는 어느 특정 단일 전략에만 의존하는 것이 아니라 다양한 적응전략을 혼용하고 통합적으로 구사하고 있었고, 그 결과로써 ‘역할갈등과 의식의 확장’의 범주가 확인되었다. 이러한 연구결과는 간호사의 의사결정이 개인의 실무경험 뿐만 아니라 간호실무가 이루어지는 사회적 맥락의 특성에 의해서 달라질 수 있음을 시사하고 있다.

Purpose: The purpose of this research was to describe the decision making of nurses in a VIP ward.
Method: The methodology of collecting and analyzing the data was based on the grounded theory of Strauss and Corbin (1998). The data were collected through an in-depth interview, which were audio-taped and transcribed. The data were collected from 10 nurses from July to November 2007.
Results: The core category on VIP ward nurses’ decision making was named as “adjusting with flexibility and deepened insight.” The causal condition was established by ‘the patients who wanted to be treated specially.’ The contextual conditions included ‘caring patients from various departments,’ ‘differences depending on the nurses’ clinical experience,’ and ‘client-centered atmosphere in the VIP ward.’ The intervening conditions included ‘problem solving styles of nurses’, ‘attitudes of patients and family members’, ‘nurse-doctor relationships’, and ‘accessibility to information.’ It was confirmed that nurses changed their action-interaction strategies depending on the intervening conditions, thus resulted in the nurses’ role conflict and the need to expand their consciousness.
Conclusion: The result of this study indicates that nurse’s decision making depends on their experiences and the nature of social context in which nursing occurs.
